What bad things may have been done to you by a student (or students) in this school? Circle an item if it was a problem for you in the past month of school.

  1. Hitting, pinching, or tripping me.
  2. Pulling my hair or biting me.
  3. Spitting on me.
  4. Hiding my things to be mean to me.
  5. Tearing my clothes, or breaking my things (such as pencil breaking).
  6. Stealing from me.
  7. Gossiping (saying bad things) about me.
  8. Writing mean notes about me.
  9. Leaving me out of a group or not having friends at school.
