Sample Data Collection Plan
The data collection plan should include when information will be collected, where it will be collected, who will collect information, and how information will be collected. The plan can be written in text form describing objectives, data elements, data sources, data collection instruments, and times to collect data. Another option is to set up a matrix with this information. A matrix can give you a snapshot of tasks and enable you to track the schedule.
Here are samples of partial data collection plans. A full data collection plan would contain additional objectives.
